The Ins and Outs of Lab Fee Reform: Urgent Details on Lab Test Payment Under Medicare April 23, 2014, 2 p.m.-3:30 p.m. In the biggest change to how laboratory tests are paid under Medicare since the Clinical Laboratory Fee Schedule (CLFS) was established in 1984, Congress has just enacted a new system for tying Medicare payment for lab tests to rates paid by private payers. The new law establishes a new market-based system for determining payment, overriding plans by the Centers for Medicare and Medicaid Services (CMS) to make adjustments based on “technological changes.” Under this new law, Medicare payment rates for lab tests will be based on a weighted median of rates paid by private payers. Any payment reduction under this new system would be capped in the first few years, and labs would have the opportunity to provide input on proposed changes.
